Nutrient Test Kit

The NTA AU/NZ nutrient test kit is a requirement for the clinical components of the Diploma. There are three options you can choose from when selecting your kit (these are listed in full on the Clinical Components section of our website). Nutrient test kit prices are listed below. All prices are in AUD, and exclude GST (applicable only to Australian residents), and exclude shipping costs. Shipping costs will vary depending on your chosen kit size, and shipping address. Shipping costs will be arranged with each student in term 2 prior to any kits being shipped. You can opt to bundle the nutrient test kit into your tuition payment (full tuition or payment plan), or you can purchase the kit separately in term 2 when the ordering window opens. If you are opting for a NTA AU/NZ payment plan, the cost of your chosen test kit will be included in the first three monthly payments on top of the tuition payments. Nutrient test kits are nonrefundable once orders are placed with the Supplier, and once ordered are not eligible for any refund under the student withdrawal policy.

Workshop Preferences *

At NTA AU/NZ honouring each person’s bio-individuality is our most important philosophy, and as such we respect everyone’s right to health privacy and their own unique needs. As an organisation we are committed to inclusivity, honouring human rights and respecting each human sovereign being. As such we now provide the following options regarding workshop attendance: Workshops are now run in a hybrid format, with the following options available regarding attendance: • In person (main centers) - this is subject to numbers, personal health requirement preferences and current travel/ health mandate restrictions. • Regional small groups - this is subject to localised support and personal health requirement preferences. • Virtual groups - no travel requirement, students will all require a practice body (i.e. family member/friend) for all days of workshops. For our team to establish the workshop formats for the cohort you are applying for, please indicate your preference for your own workshop attendance. Note: Workshop attendance is still mandatory regardless of which format you partake in.

FNTP Diploma tuition is $14,444 AUD *Tuition is in AUD and any additional costs from conversion/transaction fees are the responsibility of the student. Payment plans are available to all students and tuition can be split over six or twelve monthly installments. Each payment plan incurs a non-refundable establishment fee (see below for fee's). Payment plan students will receive an invoice on the 25th of each month with a link to pay the tuition amount. Payments are due by the 27th of the month, failure to pay by the due date will result in disconnection from the learning platform until payment is made. I authorise NTA AU/NZ staff or instructors to contact emergency services on my behalf in the event of a medical emergency during my participation in the Functional Nutritional Therapy Practitioner (FNTP) program, including during workshops. Registration Agreement *

REGISTRATION TERMS Registration is binding only once this form is completed, the student handbook is signed and full tuition payment is received by NTA AU/NZ for the selected training(s). NTA AU/NZ will provide written confirmation of completed registration. CANCELLATION/REFUND The cancellation of registration must be submitted in writing. Refunds are determined by the schedule outlined in the Student Handbook. Refunds are only applicable up to 90 days from class start date, and only to tuitions paid in full in advance, not with a 12 month payment plan. Students on a 6 month payment plan will be refunded any portion of the payments made that extend beyond the current access at the time of the withdraw, unless the 90 day withdrawal period has lapsed. Payment plan establishment fee's are non-refundable. NOTICE OF FINANCIAL OBLIGATION The student acknowledges and accepts that any contract for training they enter into with the Nutritional Therapy Association AU/NZ contains legally binding obligations and responsibilities. The student understands and accepts that repayment obligations will be placed upon them by any loans or other financing arrangements they enter into as a means to pay for their training. The student understands that any enrollment contract entered into will not be binding or take effect for at least five days, excluding Sundays and holidays, following the last date such a contract is signed by the Nutritional Therapy Association and the student, provided that the student has not entered class sooner. TUITION Your tuition fee of $14,444AUD includes: • Non-refundable $250AUD registration administration fee. • Three multi-day hands-on workshops at the end of each term. • Video and audio lectures and course materials via the NTA’s online learning system. • Q&A calls with Lead Instructors weekly, Instructor office hours and student forums Tuition does not include: • Required texts (official list will be provided prior to the start of class). • Clinical tools (including a stethoscope, blood pressure cuff) • Nutrient Test Kit • Portable massage table • Travel costs for the mandatory workshops (e.g. airfare, hotels, rental cars, parking, etc.) By signing below, I acknowledge that I have read and fully understand the terms stated above.
